Scheduling your Appointment

New Clients!

To help create a smooth, safe, and personalized experience, all new clients are asked to complete a brief registration form prior to scheduling services. This allows me to get to know you and your pet, understand your needs, and ensure that everything is properly prepared for your visit.

What you can expect:

  • The form will ask for basic information about your pet, your primary veterinarian, and the services you’re interested in

  • You may be asked to upload a current rabies certificate for your pet

  • Depending on the services selected, additional consent sections may appear

  • Everything is completed and signed electronically — no printing required

Once you’ve completed the form, we’ll be in touch within 24–48 hours to schedule your appointment!
